Health Care Professionals

Health care professionals (HCP) are essential to helping the division determine if a medical condition will affect a person's ability to drive safely. To assist the health care professionals, the Utah Medical Advisory Board (MAB) created the medical standards and guidelines.

Health care professionals are responsible for making recommendations and appropriate information related to driving safety available to their patients without reservation. A health care professional who becomes aware of a physical, mental, or emotional impairment that appears to present an imminent threat to driving safety and reports this information to the division in good faith has immunity from any damages claimed as a result of making the report.

We appreciate the role health care professionals play in providing public safety to the citizens of Utah.

What is the best way to submit a medical or vision form?
There are three (3) ways you can submit a form. The preferred method is through the medical portal. Forms can also be faxed to 801.957.8698, or emailed to dlmedical@utah.gov.

Who should I refer my patient to if they have questions about if their form was received?
Have them call our customer service at 801.965.4437 or toll-free at 888.353.4224 Monday-Friday, 8:00 am - 5:00 pm (excluding holidays.)

Does my patient need to sign the form?
No, signing the form helps us determine who the form belongs to, it is not required.

Can I use a generic form even though my patient's information is not on it?
Be sure to put your patient's name and date of birth on it, and we will accept it.

My patient says I am supposed to mark a specific category, but he does not know which one.
We recommend filling out every category you are following your patient for in that case, so we don't have to send it back when the requested category is missing.

Can I recommend a specific review time?
Yes, you can. However, if the review period seems odd in comparison to the guidelines, we may reach out to you to learn more.

Can I recommend that someone see another health care professional?
Yes, especially if you feel a more specific assessment is required.

How long does it take for a medical or vision form to be added to a driving record?
It depends on how the form is sent. If it is by the medical portal, the forms are added the same day. We receive many fax errors daily, faxing is not the most reliable, and we might never receive it by fax. Email and fax have a turnaround time of two (2) to three (3) business days.

I have a patient who is not safe to drive, so how do I notify the division that they should stop driving?
If you submit a medical form and mark level 8 or a vision form and mark level 10. You may also submit a dangerous driver form. The dangerous driver form does not need to be notarized when it is filled out by an HCP.

Can I fill out my own form?
No, a health care professional may not fill out the form for themselves.
