  1. Fill out online application

    Everyone has to fill one out - do it online and you won't have to do it in the office.

    After you fill out the application, you will be able to make an appointment.

  2. Make Appointment

    • Schedule an appointment to visit a driver license office
    • Walk-ins wait a lot longer!!
  3. Gather Your Documents

    • Bring your current Utah driver license.
    • Bring two (2) documents providing proof of your Utah address if you have moved from the last known address on your Utah driver license.  You can avoid providing proof if you submit your address change online prior to your scheduled appointment.
  4. Go to Your Appointment

    • Have your picture taken
    • Take an eye test
    • Show documents
    • Pay $52.00 fee ($27.00 for 65 and older)
    • If required (because of expiration, suspensions, or multiple citations), you will then take the 25-question open-book written knowledge test.

Get Your License in the Mail

  • A temporary license will be issued the same day if all requirements are met.
  • The completed license with an 8-year expiration will be mailed to you in approximately 4-6 weeks.
