  • If your CDL expires in the next six (6) months, you are eligible to renew it.

Lost or stolen
  • You may want to file a police report if your CDL has been stolen. 
  • If you have a police report bring it with you.

Documents for name or address change

  1. Fill out online application & make appointment

    • Currently, our Farmington, Logan, and Tooele offices do not process CDLs.

  2. Gather your documents - must bring to appointment

    • Bring your current Utah commercial driver license, if it isn't lost or stolen. 
    • Bring your current lawful presence identity document.
      • A S.A.V.E. verification will be run to verify your document status (this process may take up to 14 days.)  To understand S.A.V.E verifications, click here.
    • New address? Bring 2 documents providing proof of your Utah address.
    • Federal regulations require that your current address be printed on your commercial driver license.
    • Name change? Bring proof of your new name.

  3. At your appointment

    • Have your confirmation number ready to check-in
    • Have photo taken
    • Present documents
    • Take eye test
    • Pay non-refundable fee of $23.00

When do I get my CDL?

  • A temporary CDL with a photo will be issued on the same day.
  • The completed CDL with the same expiration date as your previous CDLwill be mailed to you in approximately 4-6 weeks.
