Lost or stolen
  • You may want to file a police report if your ID has been stolen.

Documents for name or address change

  2. Gather your documents - must bring to appointment

    • Bring your current Utah commercial driver license, if it isn't lost or stolen. 
    • Bring your current lawful presence identity document.
      • A S.A.V.E. verification will be run to verify your document status (this process may take up to 14 days.)  To understand S.A.V.E verifications, click here.
    • New address? Bring two (2) documents providing proof of your Utah address.
    • Federal regulations require that your current address be printed on your commercial driver license.
    • Name change? Bring proof of your new name.

    • Have your confirmation number ready to check-in
    • Have photo taken
    • Present documents
    • Pay non-refundable fee of $23.00

When do I get my ID?

  • A temporary ID with a photo will be issued on the same day.
  • The completed ID with the same expiration date as your previous license will be mailed to you in approximately 4-6 weeks.
