Required documents for non-U.S. citizen with lawful status
You'll need documents from each of the 4 categories below:
One (1) proof of your identity
Acceptable documents include:
- Unexpired Employment Authorization (EAD) issued by DHS, form I-766 or form I-688B verified by the Systematic Alien Verification for Entitlements System (SAVE)
- Unexpired foreign passport with a valid visa and I-94 verified through SAVE
One (1) proof of your legal/lawful presence
Acceptable documents include:
- Unexpired immigrant or nonimmigrant visa status for admission into the United States issued by the U.S. Federal Government
- Pending or approved application for asylum in the United States
- Admission into the United States as a refugee
- Pending or approved application for temporary protected status in the United States
- Approved deferred actions status
- Pending application for adjustment of status to legal permanent resident.
- Covered humanitarian parolee
One (1) proof of your Social Security Number
Acceptable documents include:
- Social security card issued by the U.S. government that has been signed (stub not accepted)
- If you don't have your social security card, you can use one of these documents instead, as long as it has your name and complete social security number
- W-2 form
- SSA-1099 form
- Non SSA-1099 form
- Pay stub
- Letter from the Social Security Administration indicating ineligibility to obtain a social security number as a result of legal/lawful presence status
Two (2) proofs you live in Utah
Acceptable documents include:
- Bank statement
- Court documents
- Current mortgage or rental contract
- Major credit card bill
- Property tax notice
- School transcript
- Utility bill
- Vehicle title
Proof of residency is required for all first-time licenses/ID cards/permits.
Proof of residency is required for all renewals and replacements if the address is different from what is on the current license record.
Documentation must display your name and address, and must be dated within 90 days.
Documents must be:
- Original documents or certified copies with raised seal, stamp, or signature
- Unexpired
Documents cannot be:
- Photocopies
- Faxed copies
- Laminated
Bring all required documents to your appointment
- You will have to return with them later if you don't
Document approval may take more time
- It may take more than a day
- It may require an additional visit
- If your document(s) aren't in English, you must have certified translation(s).
- A translator approved by the division must do the translation.
- The original and the translation count as one document for identification, so be sure to bring both.

If your name has changed
- If the name on your license or ID card doesn't match the name on your birth certificate, amended birth certificate, or passport, you'll need legal papers to explain the difference.
- These papers could be:
- A certified marriage certificate from the state
- A divorce decree
- An updated passport
- A new birth certificate
- Adoption papers
- A citizenship or naturalization certificate
- Court papers showing the name change, sealed by the court.
