When applying for a replacement license, the expiration date of the new license will be the same expiration date as your current license. However, if your license expires in the next six (6) months, you would be eligible to renew.
- You must be at least 21 years of age to apply for a taxi endorsement.
- In Utah, a taxi endorsement is not required to drive a taxicab.
- You will be required to pass a 25-question open-book written knowledge test.
It is necessary that all private passenger carriers obtain a taxi endorsement if they are operating a Class D vehicle to transport passengers for hire. Private passenger carriers are individuals who operate any motor vehicle for hire that is designed to transport 15 or fewer passengers, including the driver, and is operated to transport an employee of the person who hires the motor vehicle operator. All drivers who operate a motor vehicle on behalf of a business to transport employees must obtain a taxi cab endorsement. The definition of a private passenger carrier does NOT include a motor vehicle driven by a “transportation network” as defined in UCA 13-51-102.
- Fill out an application and schedule an appointment to visit a driver license office.
- When you arrive for your scheduled appointment, have your confirmation number ready to check in, and then you will take a photo.
- After your photo, you will have a seat; when your number is called, you will present your documents to the examiner, take an eye test, and pay the non-refundable fee of $23.00, plus the taxi endorsement is $14.00.
- You will then take the 25-question open-book written knowledge test.
- Bring your current Utah driver license.
- Bring two (2) documents providing proof of your Utah address if you have moved from the last known address on your Utah driver license. You can avoid providing proof if you submit your address change online prior to your scheduled appointment.
If the name on your driver license or identity card is different than the name on your identity document (e.g., birth certificate, amended birth certificate, or passport), it must be accompanied by legal documentation authorizing the name change. These include a certified state marriage certificate, divorce decree, updated passport, amended birth certificate, adoption paperwork, citizenship/naturalization certificate, or court documents indicating the name change sealed by the court.

When Do I Get My License?
- A temporary license with a photo will be issued on the same day.
- The completed license will be mailed to you in approximately 4-6 weeks.
Attention: By law, you may not possess more than one REAL ID-compliant certificate at a time (e.g., a Utah license and a Colorado ID.)
