Required documents for parolee or undocumented immigrant with legal status
You'll need documents from each of the 4 categories below:
Two (2) proofs of your identity
One (1) must be one of the following:
- Foreign birth certificate or unexpired foreign passport, including a certified translation if the document is not in English.
- Unexpired Employment Authorization (EAD) issued by DHS.
One (1) must be one of the following:
- Church records
- Court records
- Driver license
- Employee ID
- Insurance ID card
- Matricular consular card (issued in Utah)
- Mexican voter registration card
- School records
- Utah Driving Privilege Card (DPC)
One (1) proof of your Social Security Number or Individual Tax Identification Number (ITIN)
Acceptable documents include:
- Social security card issued by the U.S. government that has been signed (stub will not be accepted.)
- If the social security card is not available, you may present one of the following documents that includes your name and full social security number:
- W-2 form
- SSA-1099 form
- Non SSA-1099 form
- Pay stub
- Individual Tax Identification Number (ITIN) issued by the Internal Revenue Service (IRS.)
- Document or letter from the IRS verifying the ITIN.
Two (2) proofs you live in Utah
Acceptable documents include:
- Bank statement
- Court documents
- Current mortgage or rental contract
- Major credit card bill
- Property tax notice
- School transcript
- Utility bill
- Vehicle title
Proof of residency is required for all first-time licenses/ID cards/permits.
Proof of residency is required for all renewals and replacements if the address is different from what is on the current license record.
Documentation must display your name and address, and must be dated within 90 days.
Fingerprint card and photo
- You will need to have your fingerprints and photo taken by an approved vendor or law enforcement agency.
- Bring the fingerprint card and photo to your appointment.
Documents must be:
- Original documents or certified copies with raised seal, stamp, or signature
- Unexpired
Documents cannot be:
- Photocopies
- Faxed copies
- Laminated
Bring all required documents to your appointment
- You will have to return with them later if you don't
Document approval may take more time
- It may take more than a day
- It may require an additional visit
- If your document(s) aren't in English, you must have certified translation(s).
- A translator approved by the division must do the translation.
- The original and the translation count as one document for identification, so be sure to bring both.

If your name has changed
- If the name on your license or ID card doesn't match the name on your birth certificate, amended birth certificate, or passport, you'll need legal papers to explain the difference.
- These papers could be:
- A certified marriage certificate from the state
- A divorce decree
- An updated passport
- A new birth certificate
- Adoption papers
- A citizenship or naturalization certificate
- Court papers showing the name change, sealed by the court.
